The following questions were sent via e-mail on February 27, 1999, to the State of Oregon, Department of Agriculture. On March 2, the answers shown below were provided via e-mail by Dan Blevins, Pesticides Division.
- What is the name and address of Oregon’s pest inspection/pest control licensing and regulating agency?
Licenses, regulates, and investigates pest control operators in Oregon:

- How many years does Oregon’s licensing and regulating agency maintain records of complaints against pest control/pest inspection companies?
Oregon Department of Agriculture maintains records of complaints for 20 years after resolution of a case.
- How many Terminix branches are currently operating in Oregon? (My records indicate there are currently six Terminix, aka Terminix International, branches operating in Oregon.)
As of this date (3/2/99), Oregon Department of Agriculture records indicated that there are three Terminix International CO LP operating in the State of Oregon. They are based from the following cities:
- Wilsonville, OR
- Maridian, ID
- Bend, OR
- The Eugene, OR store combined with the Wilsonville, OR store recently.
If you are aware of other Terminix stores operating in Oregon we would like to know!
Not counting the Maridian, ID or Eugene, OR branches, my records (taken from the current yellow pages on the Internet) show 6 branches; however, two listings are at the same address, leaving five listings versus Oregon’s two listings:

To their credit, the State of Oregon questions any discrepancy. (Are the remaining three branches unlicensed, have their licenses been revoked, did they go out of business, or what?)
- Does each Terminix branch in Oregon operate under a separate license or do all the Terminix branches operate under one license?
All Terminix branches could be licensed under one Oregon Pesticide Operator License, if they were all Terminix Company (Memphis, TN) owned stores. If they are independent franchise owned stores, then another Pesticide Operator license would be required.
- How many complaints, if any, have been filed in Oregon against Terminix International (and its aka’s); including the nature of the complaints, name and location of Terminix branch involved; and disposition of complaints (dismissed, settled, fine, suspended or revoked license, probation, etc.)?
There were four complaints in Oregon concerning Terminix stores in the last three years; one was not investigated because it was a civil matter; one was not investigated because no application occurred; one was referred to another agency; and one was investigated with no violation found. Copies of any of the above cases can be provided if requested in writing and at a cost of 15 cents per page.
