accessible housing, home care, changes of residence,
placement in other appropriate programs and
facilities.
In Marysville and Yuba
City, social workers at the hospitals and the skilled
nursing facilities and adult caseworkers at the Welfare
Departments function as Case Managers, mainly with the
clients served by their agencies. Their assistance is free
to those they help, and most will try to find time to answer
any questions they can for people who call them. Public
Health Nurses and several Home Health Agencies can offer
assistance to families, too.

assessment, referral and consultation services. Also assists
with education to caregivers and professionals. Maintains a
gerontology library and assists families in problem solving.
Full care management services are available. Also manages
telephone reassurance and home visiting programs in
Sacramento area.
