services are available right here in our own area.
When necessary, referrals are made to the nearby
major metropolitan areas. Many area doctors accept
Medicare assignment. If you have questions, ask
your doctor about his policy. Some doctors accept
Medi-Cal on an individual basis. Many doctors are
affiliated with managed care plans and HMOs. Ask
your doctor or your insurer for the list of
participating physicians in this area.

Call the Yuba-Sutter-Colusa County Medical Society at
673-6894 for the names of physicians who are accepting new
patients, Medi-Cal patients and those who accept Medicare
assignment, or call the doctor's office and ask.
Please call the Medical Society if you have any questions
regarding a physician. To locate physicians who are not
affiliated with the medical society, consult your telephone

National Eye Care Project

Services Provided:
8 to 4 Daily. Each qualified caller will be matched with
a nearby opthalmologist who has volunteered to provide
medical eye care for disadvantaged elderly at no cost to the
patient. For those with Medicare, Medicare payment is
accepted as payment in full. For those with no medical
insurance, care is free.
Eligibility Requirements:
Age 65 or older, US Citizen or legal resident, without
access to an
eye physician or surgeon.

department and 24-hour physician-staffed emergency room.
Cardiology, CAT Scan.
Social Services workers at both hospitals offer discharge
planning and case management for hospitalized Seniors and
their families. Assistance includes placement in sheltered
living situations or finding in-home caregivers for
recovering people as well as other services found to be
needed. There is no charge for the services of the Social
Services Worker.

Services Provided: Open 7 days a week, 365 days per year
from 7 a.m. to 7 p.m., for medical care and treatment.
Fees Vary .Accepts Medicare Assignment and will bill most
insurances. Does not accept Medi-Cal. Helpful when you do
not have a regular doctor and you need to see someone right
away, though not an emergency.

FRHG is the administrative service for Rideout and
Fremont Hospitals and the Fountains. Rideout Hospital is the
location of 24 hour emergency services. Social Services are
available at both hospitals. It has full home-health service
which is covered by Medicare and Medi-Cal. Services include
home visits by licensed professionals. Home medical
equipment is available to rent or purchase. Home infusion
services provided. FRHG is the parent organization for
Valley Hospice.

Social Services include Social Worker and Discharge
Planner. Site of DP/skilled nursing facility for additional
recovery and therapy for seniors who need transition between
hospital and home. Provides physical therapy, occupational
therapy, speech therapy. Qualifies for Medicare coverage.
Social workers work to evaluate needs of the families, work
with community agencies to meet needs.
